Real documentation for the accommodations you need.

Many employees know they need accommodations at work but get stuck at the documentation step. HR asks for a letter from a licensed clinician. The clinician asks what HR needs. The accommodation request sits in limbo while the demands of the job keep coming.

An ADA Workplace Accommodation Evaluation is a clinical assessment built for the accommodation request process. It names the diagnosis, explains the functional limitations in the work context, and identifies specific reasonable accommodations under the Americans with Disabilities Act, written up in a clear, professional report you can submit with your request.

A previous accommodation request was denied for lack of documentation
Performance reviews are starting to reference issues a diagnosis would explain

You need specific accommodations named, not a vague letter
You want a clinician with experience supporting the accommodation process
If any of this is where you are, this evaluation was designed for you.

ADA Evaluation Questions

How much does an ADA workplace accommodation evaluation cost?
A flat $550, all-inclusive, with no surprise fees. That covers your clinical interview, functional assessment, screening tools and scoring, the written report formatted for HR, and your feedback session to review it.
Do you bill insurance?
No. The evaluation is a self-pay service. Payment is due at booking, and you receive a paid receipt for your records.
Is the evaluation done online?
Yes. The entire evaluation is virtual and available to adults in Oklahoma, Colorado, Texas, and Florida. You complete everything from home, wherever home is in those four states.
What does the report look like?
Your report includes the clinical methods used, a review of your history and presenting concerns, screening results, diagnostic impressions based on DSM-5 criteria, an assessment of functional limitations in the work context, and specific recommended accommodations. The report is clear, professional, and structured so you can submit it directly with your accommodation request.
What kinds of accommodations are typically recommended?
The recommended accommodations depend on the diagnosis and how it shows up in your work. Common examples include flexible scheduling, written instructions instead of verbal, a quiet workspace or noise-canceling support, modified meeting structures, additional time for tasks, the ability to work remotely on certain days, breaks during the day, and adjustments to communication expectations. Your specific recommendations come from a careful look at what would actually help you do your job.
Will my employer have to approve every accommodation?
Under the ADA, employers are required to engage in an "interactive process" with employees who request reasonable accommodations. They aren't required to approve every specific accommodation, but they are required to seriously consider the request and either approve it or propose an alternative that addresses the same limitation. A well-documented evaluation report strengthens your position in that interactive process.
Can you guarantee my accommodations will be approved?
No clinician can guarantee an employer's decision. What this evaluation provides is a thorough clinical assessment and a professional written report supporting your accommodation request. Your employer makes the final decision under the ADA interactive process. What you'll have, regardless of outcome, is a clear, professional document to bring to the conversation.
Do I need to share my diagnosis with my employer?
Generally, employers are entitled to know that you have a qualifying disability and what accommodations you need, but they aren't entitled to your full medical record. A standard accommodation evaluation report shares only the diagnosis, functional limitations, and recommended accommodations, which is what HR needs to engage in the interactive process. You can ask your therapist about what to share in your specific situation.
What conditions does this evaluation cover?
The evaluation can address a wide range of mental health and neurodevelopmental conditions that affect work, including ADHD, autism, anxiety disorders, depression, PTSD, OCD, and others. If you're not sure your concerns qualify under the ADA, mention them when you book and we can talk through whether this evaluation is the right fit.
How long does the process take?
Plan for two virtual appointments: your clinical interview and your feedback session. Your written report is delivered within 5 to 7 business days of completing your questionnaires.
